Case Report on Choledocholithiasis with ERCP with thrombosis of the portal vein, splenic vein, superior mesenteric vein with systemic

Background: Cholelithiasis, one of the most common medical conditions leading to surgical intervention, affects approximately 10 % adult population in United States. Choledocholithiasis develops about 10%-20% patients with gallbladder stones and literature suggests that at least 3%-10% undergoing cholecystectomy will have bile duct (CBD) stones. Case Presentation: A 48-year female was admitted tertiary care hospital Wardha medicine ward, primary complaint abdominal pain, back nausea, vomiting, fever, bloating abdomen, constipation. No history such as diabetes mellitus, significant history. Physical examination systemic were done. The patient's general appearance is not good, she undernourished, patient active very dull nature, maintained hygiene personal grooming Conservative. Haemoglobin gm%, WBC Count 5000 cells /mm³, RBC 20000- 40000 IgM IgG /mcL test both positive, platelets count 12000 / mm³, endoscopic retrograde cholangiopancreatography for confirming diagnosis Choleodolithiasis ERCP thrombosis portal vein, splenic superior mesenteric vein systemic. Computed tomography Magnetic resonance imaging, urine culture, complete blood done which abnormal. transfused surgery ward management. Patient managed laparoscopic help remove gallstones also management given antibiotics, analgesics, antacids, other supportive measures. Conclusion Due conservative quality nursing care, condition stable had no complaints present hence being discharged.
